4090 was a production Metrocar built by Metro-Cammell at Washwood Heath, Birmingham.

4090 and 4040 were flooded on April fools day (the 1st of April) 1992 and we're trapped in the flood for a day at Airport with the water subsiding on the 2nd with the flood being caused by the flood pumps failing

4090 along with 4010 were involved in an incident on the 11th of August 2014 at Walkergate where an arc flash happened that cause a lot of sparks to be emmited from the overhead lines with it melting about 18 seconds later and start to violently flash on the roof of 4090 and the sparking and heat from the wires caused the electrical equipment in 4090 to catch fire. 30 people were on board thankfully nobody was on the platform or injured as 4010 and 4090 were ready to depart with the doors already shut. Both units retured to service not long after.
